Narrative

IGNITION - Occurred on the surface at Deep Well Pump #18 Metal casing installation, as a threaded piece was being welded on top of the metal casing. The flame was blue and orange in color, lasted approx. 7 to 8 seconds and was extinguished by a fire extinguisher.
