Narrative

Employee was running the nozzle end shooting shot-crete when the nozzle plugged, employee lifted full face mask to tell partner to shut off the air, the nozzle blew off the hose causing shot-crete to contact the employee's face and eyes. Seen by site medic, referred to ER and then to eye specialist, restrictions were issued
