Driller was lowering core tube retraction tool (also known as the overshot tool) down into the cased hole. As the driller was returning the overshot tool to its resting location, EE inadvertently tripped hydraulic lever to the ""up"" position, rather than the ""down"" position, which pulled the driller's arm upwards.
