## Narrative

After installing bolt off miner in 6E #3 entry, just outby 9 crosscut, the injured was walking toward the tail of the miner when a piece of rock fell from the roof and struck head. When ee reported to work on 5/30/18 ee reported the incident and went to seek medical attention, being diagnosed with a concussion.
