Narrative

Employee was using wrench to install rollers on portable plant. Felt a ""pop"" in rt shoulder. Refused medical treatment until 05/16/18. Placed on light duty. No light duty available. Returned to regular duty 05/22/18. Follow-up with dr 5/31. Placed on light duty. No light duty available.
