Narrative

Employee was bolting in 5W Belt, when drill steel that was sitting against rib fell striking emp on left arm causing laceration. Injury resulted in 9 sutures. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor
