Narrative

While dragging a ventilation tube to the face, the employee stepped up over the coal pile behind the miner. The coal moved beneath EE's left foot causing EE to twist EE's left knee. The employee did not violate any laws or policies, and was wearing the required PPE at the time of the incident.
