Narrative

Employee was loading roof bolts onto the Roof Bolter. While doing so, a piece of mine roof approximately 8"" x 8"" x 2"" fell and struck EE on the right middle finger causing a laceration that required part of the fingernail to have to be removed and 2 stitches to close the wound. The mining height is 8 1/2 feet in this area.
