## Narrative

Bolting off right side of miner in St Leo while cutting a corner in 3-4 crosscut. Injured had installed a roof channel and was walking toward the back of the miner when a rock fell from between the straps (4'x2'x4"" thick) striking the top of their head and knocking them into the mining machine, resulting in a lost tooth and minor abrasions.
