## Narrative

An unplanned roof fall was discovered at the 18 block intersection of the 6 North Belt. The fall was approximately 16' wide, 64' long and 10' high.  The fall was most likely caused by gradual deterioration of the intersection. No problems with mining machines, PPE, training or compliance issues.
