Narrative

EE was taking off the #2A unit dust collector to change out the 2A Short head. The EE had removed all the connecting bolts for the dust collector pipe with the exception of two, one at each end. The dust collector pipe fell approximately 4 inches hitting EE on EE's hardhat and pinning EE's left forearm between the wall, existing conduit with 24 inch dust collector pipe.
