## Narrative

The stab jack pin broke on the operator side drill station canopy. The roof bolter operator set the jack on the ground in order to begin bolting. As a result of the broken pin, the canopy lowered when bolter operator pressurized the stab jack. The canopy lowered on operator's head resulting in a strained neck and shoulder.
