Narrative

Employee bolting SR punch through, a section of rock rib above the coal rolled off striking employee. Rock was 12' L, 2' W and 10"" thick. Legs were trapped under one end of rock. Employee complained of leg pain and then back pain as employee was being removed from mine. Employee has fractures to right ankle, both sides of pelvis, spine and ribs.
