Narrative

Employee was attempting to unplug a pressurized rock dust fill hose. As they lifted the hose, the pressure released and caused it to whip and strike them in the right elbow. It did not involve job skills, proficiency, training, PPE or protective devices. The employee returned to work on 7/23/18 (after vacation) and continued to work until 9/12/18.
