## Narrative

The employee was prying a cover off of a stage loader crusher, that was being held up by come-a-longs. When the come-a-longs got the full weight of the cover, the come-a-long came loose from the top and the employee was caught between the crusher cover and a pile of cable laying on the pavement behind EE. The employee was wearing all the appropriate PPE.
