## Narrative

Employee noticed the bit was wobbling in the jack hammer and then turned the hammer upside down to inspect finding the bid had broke.  Employee removed the protruding part of the bit at which time the hammer was sat down accidently firing the mechanism.  When it fired it shot the inner piece of broken bit out hitting employee on the bridge of the nose and safety glasses.
