## Narrative

IE stated picked up a bundle of bolts, began to turn towards the hauler, feet got tangled up on a pallet of supplies and started to fall. During the fall, IE tossed the bolts and tried to catch balance. Left arm/shoulder twisted and rolled in a backward motion. IE was brought out of the mine, taken medical facility & arm placed in a sling, with no use of left arm.
