## Narrative

An employee was taking slack out of a chain that was attached to a hook that was welded on a scoop bucket. The chain was ran through a continuous miner head in an effort to pull the head out. While reconnecting the chain to the hook, the miner head shifted causing the chain to catch the employee's left index finger between the hook and the chain resulting in a broken finger.
