Narrative

Employee was lifting rocks off of the mine floor and putting them on a conveyor belt and soon after felt a pain and numbness in the shoulder/arm. The belt line is typically around 42"" high. EE recently sought medical treatment and was diagnosed with a disc herniation and went off work on 12/28/2018 pending surgery.
