## Narrative

The employee and 2 others were replacing a 1.5"" ball valve. Once completed, the valve was energized & the employee tried to open the valve. The entire pipe assembly started rotating. The valve was isolated & the employee tried to tighten the assembly by hand. The valve was re-energized & the employee was struck by the assembly rotating off the header pipe extension.
