Narrative

Alleged employee stated soreness in left shoulder multiple times over past year, each time no specific event reported. Employee had previously been involved in non-work related motor cycle accident. On August 21, 2018 employee reported EE felt a strain in left arm while pulling on chain of chain hoist. Employee had surgery for left rotator cuff tear on January 07, 2019.
