Narrative

Employee unplugging Hydrator, plugged due to power outage. Using pipe wrench to move the first stage of the hydrator, wrench broke. Employee caught self to regain balance, pulling shoulder. Employee was initially seen by doc., returned to work no restrictions. Follow up visit identified weakness, determined to be from tear in rotator cuff. Employee had surgery on 1/9/2019.
