IE contacted Mach HR and Safety department on 06/26/2018 concerning pain in IE's left elbow and wrist due to repetitive motions from a work history consisting mainly of roof bolting. IE received therapy treatment for carpal tunnel. IE had surgery for carpal tunnel on 01/24/2019.
