Narrative

EE stepped over the mill motor's tracking rail and slipped on ice which measured 24'x18'. EE lurched suddenly and slightly forward to maintain balance. The tracking rail measured 3.25' tall and 4' wide. EE reported to supervisor within the hour and 2 days later ee informed supervisor that ee had significant pain on the left lower side of back.
