Narrative

IW was grouting a finished hole through a packer when the pressure started to push the packer out of the hole. IW took a 36"" pipe wrench and attached it to the packer to try and hold it from coming completely out. The packer then surged out of the hole and pinched Left middle finger between the wrench and foot clamp. This caused a laceration that required sutures.
