## Narrative

EE was installing roof bolts at the face when a piece of rock was dislodged and fell striking EE on the head causing a laceration above the left eye. EE received 3 stitches. The piece of rock was approx. 8""L x 4""W x 1"" Thick. EE was wearing required PPE, has training & experience. No rules or regulations were violated.
