A Lab Tech, was exiting truck at the Step 2 Agglomerator at line 5 and slipped on ice striking left elbow and felt pain in left shoulder. After telemedicine exam, ee returned to work without restrictions. Incident became a recordable on 2/20/19 when we were notified the employee needed Surgery
