Narrative

Injured ee was standing on the 1W Section belt tailpiece anchor supervising the alignment of a shuttle car into position with the tailpiece. EE lost balance & slipped off the support causing right ankle to roll when ee landed onto the mine floor. Employee failed to stand in a stable position. Was wearing required PPE and is experienced. No regulations were violated.
