Employee was standing on coal behind the continuous miner and tossed a ventilation tube toward the face. As EE tossed the tube, EE slipped and felt a pain in lower back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
