Narrative

Safety Dept. on-call person received a call @ 1:50 pm reporting that a manager was trapped in the Conc. Step 3 elevator. MEI was contacted & they stated they would send someone out to repair the elevator. Safety Dept. notified MSHA @ 2pm to report the entrapment and again at 4:30 when employee was rescued.
