## Narrative

Attempting to clear a jammed rock from the jaw crusher, the employee donned fall protection, and descended a ladder placed in the opening of the crusher.  The ladder slipped and twisted.  The employee's left leg got between two of the ladder rungs resulting in the left ankle being fractured.
