## Narrative

Employee stated EE was operating the #420 Fletcher CHDDR-13 Roof Bolting machine on the Unit 2 Section. In the process of drilling a hole into the top to install a roof bolt a piece of rock measuring 36"" Wide X 22"" Long X 8"" Thick fell out from between the roof bolts and struck the injured employee on top of EE's right foot.
