An unplanned roof fall was discovered in the 6 North Parallel, #4 Entry, #4 block. The fall was approximately 16' long, 16' wide, and 8' high. The fall was more than likely caused from gradual deterioration. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
