Narrative

Employee was cleaning the area around 4 mill when EE decided to move a few 55# grinding media bags off of a platform about 42"" above the floor without assistance. EE grabbed a bag in each hand and as EE swung the left handed bag to the floor, EE felt a pain and pull in EE's abdomen. Diagnosis hernia and placed on restricted duty.
