An unplanned and reportable roof fall occurred at the 14 bk, #6 entry intersection on the D-Mains #1 additional intake that leads to the D-Mains working section (MMU 001-0 & MMU 005-0). There was no equipment damage, no injuries and the ventilation system was not disrupted. The occurrence was immediately reported.
