Narrative

Installing center pin in D33 dragline. Pin is packed in dry ice before installation. When the dry ice was released from freeze box, dry ice dumped, creating a cloud of CO2. Employee became disoriented as result of CO2 exposure. Employee did not lose consciousness. Employee was transported via ambulance for medical evaluation and taken off work until fully recovered.
