Narrative

On 1/30/18 EE reported that arm had been hurting THEM. Has been for a week. Was taken to clinic to be evaluated. On 04/01/2019 was seen by specialist for continued pain. On 07/02/2019 EE had carpal tunnel surgery on right arm.
