Narrative

Employee was hand dusting rib when the roof bolt operator swung the boom in to install a middle bolt. A loose roof bolt in the tray caught on the bolter light housing and popped out of the tray striking the employee in the forehead causing a laceration. (14) sutures were required to close the wound.
