## Narrative

The inlet duct of the coal mill had smoldering material that was ignited from a combination of a blaster firing air into the system and coal dust being present in the system.  The blaster forced air into the coal and heat causing an explosion. It has also been discovered a fast acting damper which should have closed automatically did not.
