Employee originally reported OD symptoms (Carpal Tunnel) in left hand on 12/22/17 to the operator. While off on personal health issues, employee had surgery for OD claim. Surgery was on 4/23/19 while still out on personal issue. 8/14/19 EE was released on personal health issue & EE's 1st shift missed for reportable occupational disease was 8/15/19.
