## Narrative

Employee was preparing to wash spillage from under a conveyor. A new nozzle that was being used was 1.5"" thread and so was the mating threads. The threads were close enough to engage and tighten. However, the unknowing different thread type did not hold pressure resulting in the hose blowing free and striking the employee in the upper lip.
