Narrative

EE was working above chute at ground level to clear chute. Hardened material near the chute fell towards employee and EE bruised EE's left elbow/shoulder trying to leave area. EE treated with Dr. with ice and ibuprofen starting 7/8/19. The area didn't improve so employee was prescribed PT starting on 8/29/19 changing case from First Aid to Reportable.
