Narrative

Employee was rib pinning and was switching to a longer drill steel and got left index finger caught between the drill steel and the chuck causing a laceration. Employee requested medical treatment on 9/9/19 and did not become a lost time accident until 9/11/19. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
