An unplanned and reportable roof fall was discovered on 9/15/19 at the 4 Bk intersection #5 entry of the D-Mains #2 Beltline. The roof fall occurred in the alternate escapeway. The miner unit effected inby was idle on the day the fall was discovered. This occurrence was immediately reported.
