Narrative

Employee was using an index bar to pry with while removing a stab jack from a piece of equipment & felt burning sensation in both arms. Later, medical examination was sought & showed that employee needed surgery for the repair of the condition, surgery on 9/23/2019, therefore, employee is on days away from work. Late reporting due to the late medical evaluation.
