## Narrative

An unplanned roof fall was discovered in the Main North #3 entry at the 86 block overcast row. The fall was approximately 12' wide, 40' long and 12' high. It was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance.
