Narrative

Employee was trying to remove an accumulator from a hydraulic valve bank. While trying to remove super stecko staple from fitting, employee inserted pointed end of pick hammer between staple and fitting. Employee hit pick hammer with ball peen hammer to dislodge staple. Metal shard broke off and lodged in employee's left wrist.
