## Narrative

Employee was inside the outer half of the east drum and upon exiting the drum employee was straddling the uneven sump floor and attempted to pull self up and over to the hoist room floor.  In the process both hands slipped and employee fell backwards approximately 4 ft. below into the 2 ft. gap and onto the sump floor.
