Narrative

While positioning a 24"" HDPE Stub End in the fusing machine the stub end fell/slipped backwards causing Employee's hand to be caught between the stub end and fusion machine bracket resulting in the injury. Employee was unaware of hand position, employee was wearing hand protection.
