Narrative

Employee was at the Viola Shaft working on top of a 650 scoop to be hoisted outside to the surface, when employee attempted to lower self from the scoop, employee slipped, causing a sprain/strain injury to right knee. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency not a factor.
